ETO COC Assessment Documents Checklist

If you are going to appear for ETO COC examination in India then you have fulfilled below documentation thoroughly.



ELECTRO-TECHNICAL OFFICER COC Examination

1. DOCUMENTS REQUIRED FOR ELECTRO TECHNICAL OFFICER ASSESSMENT AND BOOKING.

2. ALL ORIGINAL DOCUMENT TO BE PRODUCED DURING ASSESSMENT.
                                                     
3. SELF CERTIFIED PHOTOCOPY OF EACH DOCUMENT MUST BE ARRANGED IN THE FOLLOWING SEQUENCE.

A. WRITTEN & ORAL  ASSESSMENT

REQUIRE DOCUMENTS

1          CDC including relevant pages of sea service.
2          Passport (all stamped pages)
3          INDOS Certificate
4          Educational Qualification.
5          Bridging Course / ETO Course (as applicable).
6          Approved Workshop Certificate.
7          Validity page of RPSL No. issued to the company. 
           (DGS website: www.dgshipping.gov.in)
8          Sea Service Testimonials. Indicating Sea Time and Propelling Days duly signed by Chief Engineer and Master of the Vessel.
9          The article of Agreement (Only for Indian Flag vessels above 200 GRT)
10        Sea Service Extract ( Only Navy Candidate)
11        STCW Course as follows:-  
            i)          Personal Survival Technique (PST) & PSCRB*
            ii)         Elementry First Aid (EFA) & MFA *
            iii)        Fire Prevention & Fire Fighting. (FPFF) & AFF*
           iv)      Personal Safety and Social Responsibilities (STCW  1995)

* PST, FPFF, PSCRB, AFF, MFA  Refresher courses (STCW 2010) are required if the course are more than 5 years old.

12        Certificate of Proficiency in Security Training For Seafarers with Designated Security Duties (STSDSD) / Ship Security Officer (STCW 2010).
13        Valid Medical Fitness Certificate in ILO Form (DGS Approved Doctor)          
14        Seafarers Profile (Updated).
             
B) ORIGINAL OF FOLLOWING DOCUMENTS ALONG WITH THE PHOTOCOPIES FROM Sr. No. 1 to 14.

1)         Original Sea Services letter issued by the Owner of the Vessel / RPSL Agent(Addressed to PO, MMD, Mumbai).

2)         Fees for ETO Assessment and Booking Rs. 12050/- (Assessment Rs. 3000/- + Written Booking Rs. 4500/- + Oral Booking Rs. 4500/- + Bank Charges if any).

Note: Original of the above-mentioned documents to be produced at the time of Assessment and Booking for verification.
